I missed a midterm examination due to illness. What do I do?
A UOIT Medical Statement must be filled out by a physician within 24 hours of the missed midterm examination, which indicates the nature of your illness. The completed Medical Statement must be submitted to the Faculty of Science Academic Advising office within five business days in order to be considered. If the Medical Statement adheres to the above guidelines, the midterm examination will be re-weighted to the final examination. I missed a final examination due to illness. What do I do?

Two forms must be completed; a UOIT Medical Statement and an Application for Deferred Final Examinations. The Medical Statement must be filled out by a physician within 24 hours of the missed final examination, which outlines the nature of your illness. Both forms must be submitted to the Science Advising Office within five business days in order to be considered. If the documents adhere to the guidelines above, a new examination period will be scheduled at a later date. I missed a laboratory/tutorial session or an assignment/report deadline due to illness. What do I do?
A UOIT Medical Statement must be filled out by a doctor within 24 hours of the missed work, which indicates the nature of your illness. The completed Medical Statement must be submitted directly to the Instructor or Senior Laboratory Instructor within five business days in order to be considered. Consideration regarding the missed work is at the discretion of the Instructor of Senior Laboratory Instructor. The Medical Statement form can be accessed through the student forms page.

I missed a midterm or final examination due to extenuating circumstances. What do I do?
Please write a formal letter explaining the circumstances surrounding the missed midterm or final examination and submit to the Academic Advising office. In addition to the letter, if a final examination was missed, please submit an Application for Deferred Final Examinations form. Include any supporting documentation with your submission. All documentation will be forwarded to the Academic Appeals Committee for decision.

A scheduled midterm or final examination falls on the same day as a religious observance. What do I do?
The Application for Deferred Final Examinations for Religious Observances form must be filled out and submitted to the Academic Advising office. For midterm examinations, the forms must be submitted no later than three weeks prior to the date of the midterm examination. For final examinations, the form must be submitted three weeks prior to the start of the examination period for final examinations.
